At Bridgeway Community Health, our Primary Care team provides comprehensive medical services designed to support your overall health and well-being. Whether you need preventive care, treatment for an acute illness, management of a chronic condition, or coordination with specialists, we’re here to help you access the care you need, when you need it.
As Oregon’s first addiction treatment provider to offer medical services within a Patient-Centered Primary Care Home (PCPCH), we take a whole-person approach to healthcare—addressing physical, mental, and behavioral health needs in one integrated system of care.

- Oregon Health Plan (OHP) and other private insurances upon approval.
- Bridgeway also offers self-pay options as well as support with enrolling in OHP.

Coordinated Care You Can Trust
If you already have a primary care provider, our team can coordinate with your existing healthcare providers to ensure seamless care. If you do not currently have a primary care provider, we can serve as your medical home and provide comprehensive primary care services.
Should you need specialized care beyond what we offer on-site, our providers can connect you with trusted specialists and healthcare professionals throughout the community.

Prescriptions & Medication Management
To ensure safe and effective care, prescription refills require ongoing provider oversight. Refill requests submitted through your pharmacy are typically processed within 48 hours.
If you are nearing the end of your prescribed refills, please contact our office to schedule a follow-up appointment as needed.

Referrals & Specialty Care
If specialized care is needed, our providers can coordinate referrals to specialists and assist with insurance authorization requirements when applicable.
Because insurance plans vary, patients are encouraged to verify specialist coverage with their insurance provider before scheduling specialty appointments. Our team is happy to assist with questions regarding the referral process.
Hospital & Care Coordination Services
If you are hospitalized, our team can help coordinate your care, communicate with your hospital providers, assist with discharge planning, and schedule timely follow-up appointments to support a smooth transition back home.
We strive to see hospitalized patients within seven days of discharge whenever possible to ensure continuity of care and support recovery.
